There are several forms that make up the new client packet that you will need to complete. We have tried to make this process as easy as possible by having our forms available to complete online below.
If it is difficult for you to complete the paperwork online, please call us. We can complete the forms together over the phone.
Let us know if you have any issues accessing the questionnaires and forms. You can contact us using the message box below and we will provide you with the appropriate links to complete the new client packet.
NOTE: all of our questionnaires and forms ask for your email as the first question. This is done so that a copy of your responses and completed forms can be sent to you for your records.
The New Client Packet (for Massachusetts Residents)
Basic Information—name, address, phone, email, etc.
Life, Status, Health, & Mental Health assessment questions
Environmental Illness (EI) assessment questions (symptoms, history, exposures, reactions)
Do not complete this form until after the initial assessments have been completed and you and your counselor have agreed to continue working together. Typically this happens in the second or third session.
- Policy Statement to sign (usually done during 2nd session)
IMPORTANT: Do not share confidential or personal information in any communication or message you may leave.
The Counseling Center at CELA responds to mail, emails, voicemail messages as quickly as possible, typically within two to three business days.
