1. All the schools listed to provide requested records and allow for review of my application for the admission process for the certification program I have chosen on the application;
2. I confirm that all the information contained in this application, including any supplemental information and documentation, is factual, true, and honestly presented and that I am the person submitting the application; and
3. I acknowledge that I understand that (a) a $20.00 online nonrefundable application fee is required in order to process my application, (b) that CELA accepts payment as specified on the CELA website only, (c) that incomplete applications cannot be processed by CELA (an incomplete application is one where the application fee has not successfully processed and/or is missing the applicant’s signature and/or is missing the date.)
